Playing Resident Evil:4 at like 1:00 in the morning. It was raining in the game, and I turned around and all of a sudden there was some huge motherfucker with a chainsaw.

I played the first Resident Evil game at my friends house, when it first came out, scared me so much I had occasional nightmares for a long time.
I've recently been playing Gears of War, and some short moments in that game make me jump.

Ravenholm from Half Life: 2 was scary shit.
When you just begin a new level and the first thing you see is a pair of legs hanging from a tree, you know you're in for a treat.

I'd say I've never been scared while playing a videogame but the freakyiest thing that ever happened to me while playing a video game was over at my friends house, which is next to a huge graveyard, playing Guantlet Dark Legacy in the fair part of the tower with all the smileing clowns from hell and all the happy creepy stuff. That or Hello Kitty Island Adventures.

I can't think of any moments where I've actually gotten scared to the point where I've been scared in real life (I've never played Resident Evil though), but I wouldn't doubt that something in a video game has made me jump (I jump at pretty much anything unsuspecting).
But when I used to play Diablo (please keep in mind I was like 10 or 11) I would like freak out that a million guys would come around the corner and kill me (in the game). I would seriously like refuse to go in doors for like 30 seconds until my brother yelled at me enough to go inside, just because I was afraid of what was inside hahaha. (It was even worse with the doors were so big the "fight one enemy at a time and when you get low on health close the door" technique wouldn't work).

Especially Headcrabs in the vents, along with all the other creatures in that game.

Yeah, probably RE4 for me too, considering I mostly play games like San Andreas, Tony Hawk series, and guitar hero.
Umm.
It's definately not the scariest thing ever, but when i was like 10, I was playing Twisted Metal: Black, and Sweet Tooth's beginning and that cop guy's ending always scared me shitless, idky.
Also, when I was 12, I played San Andreas a lot, and I was looking for bigfoot in back o' beyond because of common myths, at about 3 am with my stereo loud so I could hear distanced noises so I could know if I had to run or whatever, and a car blew up nearby and shook my screen. It woke me the fuck up. Made me look around the area. (In-game, not in my room.)
Scariest was RE4, when I got it I'd play til like 5 AM.
Probably when I was playing the Mercenaries side-game when I hadn't slept for days because I didn't want to, I had killed almost all of them then one of the Ganados with a double-chainsaw just landed in front of me, and my stereo was blasting loud so I pretty much jumped backwards into my wall.
By what James has told me, F.E.A.R. has some shit that'll make me jump though.

Do you read Gameinformer? Because their latest issue had something like this. But anyways, I was playing Call of Duty 2 on the 360. I went on a rampage, killing all these nazis, and just when I thought I had killed em all, I turned a corner and BAM! Out of nowhere, some German came at me yelling and holding up his gun because he was about to hit me. I was scared sh*tless.

THE NOISE ALONE SCARED THE HELL OUT OF ME.
SO MUCH, IT DESERVES CAPSLOCK.
They're so much easier to beat with the infinite launcher. But they're still scary as shit to me.
My sister can't play the farm area at all, regardless of her weapons or how many times she's done it.

I have to say Clock tower 3 is the worst though... huge guy jumping out of nowhere... scary as hell! my 23 year old brother almost pissed himself... i mean we were playing... the main character was walking down the hall and out of nowhere he jumps from the ceiling and swings a giant freaking hammer at you... you cant defend yourself so you have to run and hide... but if you go into panic mode then you die

In condemned criminal origins, In an office I was walking through it and I walked up to a ledge looking over some stairs, and a Zombie jumped up, grabbed my guy, and then screamed in my face and left.
and Then in fear, I was crawling through a vent and that creepy girl came crawling at me like the girl from the ring.
